How a NDIS plan review can help

Over time, things change in your life. Which is why your NDIS plan will need to change with you.

Your plan will be reviewed from time to time.

A review is when you look over something and decide if it needs to be changed. It will help you have the right supports to meet your goals.

From 3 October 2024, the NDIS made some changes to how reassessments work. These changes make it easier to adjust your plan when you need to.

This Easy Read guide will help you change your plan.

How can you change your plan?

There are now two ways to update your plan:

  1. NDIS Plan Variation
  2. NDIS Plan Reassessment

What is a NDIS plan variation?

A NDIS plan variation is a small change to your current plan.

You might ask for a variation if you want to change:

  • A small mistake in your plan
  • Your goals
  • Your contact details
  • The date of your next reassessment
  • How your funds are managed
  • Your funding periods
  • Extra funding support if your situation changes suddenly
  • Which provider gives you support

What is a NDIS plan assessment?

A plan reassessment used to be called a NDIS Plan Review.

A plan reassessment is a full review of your plan. It can be:

  • scheduled – your NDIS plan should have a ‘reassessment date’ on it.
  • unscheduled - you can ask for a reassessment when you want.

A reassessment is needed if:

  • Your situation changes, and you need different supports.
  • You need to include the total funding amount in your plan.

Plans made after 3rd October 2024 now show the total amount of NDIS funding you have.

These plans show:

  • How much NDIS funding you have in total
  • How long your NDIS funding needs to last

How can you prepare for a plan reassessment?

Follow these steps to prepare:

Step 1: Think about your current plan

Ask yourself:

  • What’s working well in your plan?
  • Are there any supports you don’t need anymore?
  • Do you need new or extra supports?

Step 2: Talk to your support team

Your support team can help you get ready. This might include:

  • Your support coordinator.
  • Service providers.
  • Family, friends, or carers.

Step 3: Thank about your goals

What do you want to achieve in the next year?

  • Maybe you want to learn a new skill.
  • Or join activities in your community.
  • Or get extra therapy to help you feel stronger or more confident.

Write your goals to share during the reassessment.

Step 4: How is your plan being managed?

Could you think about plan management, self-managing or NDIA managing your plan?

If you have questions about plan management, talk about this at your plan reassessment.

You can decide if your funding needs to change.

Step 5: When do you want your next reassessment?

The NDIS will work with you to decide how long your plan goes for.

New and reassessed plans usually last 12 months.

Talk to your NDIA planner if you want a longer plan length.

During the plan reassessment

When it’s time for your NDIS plan reassessment, remember:

  • Be honest about what you need.
  • Talk about how your disability affects your daily life.
  • Share any changes in your situation, like new health needs.

The NDIS will work with you to create a plan that suits your life and goals.

After the reassessment

Once your reassessment is done, you’ll get your updated plan.

If there’s something that doesn’t feel right, you can ask for a plan variation to make changes without waiting for another reassessment.
